I haven't been to a ton of airports, but I'll just speak from what I know.
The best airline I've sat in is Cathay Pacific. I haven't the luxury of sitting in a ton of fancy airlines, but Cathay is no contest. Good service, excellent food, great movies, top of the line aircraft. Plus, they have power sockets for in-flight charging of electronics, which is the greatest idea in the history of aircraft and should always be utilized. I hear Singapore Airlines is pretty good too, but I don't have the luxury of sitting in that yet. The worst is Royal Brunei Airlines, which in itself is not that bad. It's just we had terrible experiences with it (lost luggage, massive cancellations, etc) , and it is rather subpar for a non low-cost airline. Also the only flight I've ever thrown up in in my life. Though I absolutely detest flying, I think most airlines I've used so far have done their job. The best airport I've been to is Changi Airport in Singapore. State of the art, simple yet classy, and has all the amenities you could want anytime of the day. It is also very well connected between terminals and to the city. The only thing really lacking is power sockets for charging laptops, but they make up for it with impressive charging stations and booths. Hong Kong International Airport comes in a close second. The worst airport I've been to is O'Hare International Airport in Chicago. It's not lacking in facilities per se, once you've passed security. However, before security almost everything is hell. No restaurants outside of the international terminal, no restaurants past security IN the international terminal, crazy long check-in lines, frightening TSA security, lack of simple facilities such as currency exchange (only one available), over-crowded terminals, not the friendliest staff, poor transport to get to other towns, and a really depressing industrial design. Let's not get started on their immigration lines. Its one saving grace is the wide array of hotels to check in during long transits but that's about it.

I don't think I've had a particularly bad experience with airliners. I have liked American Airlines (pretty average), Virgin Atlantic, and JAL. When I leave for Japan in September, I plan on taking ANA. I've heard really good things about it.
As far as airports go, I felt great at Centrair (the Chuubu international airport near Nagoya in the Ise Bay). It's pretty new and has great food options. Also, there are multiple levels of fun and free carts and transportation to local hotels, etc. Even though it was smaller than the Narita airport, it still had much to offer. The worst airport for me is a no-brainer: JFK in NYC. It's more updated than, say, the Chhatrapati Shivaji in Mumbai (which is a pretty basic airport), but the people are rude/impatient, the airport bathrooms aren't necessarily the cleanest, and you even have to pay for luggage carts. I just couldn't wait to leave JFK.

The only reason I might fly through JFK again is because flights are often cheaper through there since it's such a major airport. Otherwise, if I were you, I'd try to avoid it.

As far as airliners go, the best I've flown is Cathay Pacific. Service is great, food is delicious and the in-flight entertainment is worthwhile. And the flight attendants are gorgeous.
On the other hand, Philippine Airlines is the complete opposite. Their planes are falling apart (the air conditioning died out on my last flight with them), the food is horrible and the in-flight entertainment is outdated. Oh yeah, their flight attendants are old. In terms of airports, the best one is Hong Kong International Airport. Railway transportation to and from Hong Kong is a breeze, the airports facilities are extremely clean and organized, and it makes good use of vintage airplanes to decorate the terminals. Meanwhile, I've never been at an airport worse than Ninoy Aquino International Airport in Manila. JFK Airport pales in comparison to the Manila airport's lack of running water, insufficient lighting, useless security measures, and the number of street children harassing tourists to pay for pictures. Also, you have to pay a terminal fee for a building that's falling apart.
