Koyumi is the Philosopher's Stone.

She never spawned a phantom. She was actually the one who caused the eclipsed that spawned every other phantom.
She requires mana not to live, but to remain sentient, running out of mana will cause her to "Die" and become the stone once more, forming Haruto's final ring.

Honestly I thought this was made pretty darn obvious after the last episode.

It provides the episode 45-48 "Death" and it's a nice twist.
