in that show, Onxy was his final form...did they use he Survive modes at all?

I don't believe so. Honestly? I could never make it through the whole series to find out.

Well... They're in the Wii game so I assume that they are, but I don't plan on watching the show, so, I'm not sure.

I've not seen the show yet, but everything I've read online suggests they were used.
