Spawn was PG-13. The movie was a Saturday morning cartoon version of the comics. The movie did get a rated R director's cut that is 2 min 23 sec longer than the Theatrical Cut. However it's hardly different then the PG-13 version. Things are much stricter in 1997 compare to now.

The original cut was rated R then got toned down to be PG-13 then the original cut was re-released as the Director's Cut.

And frankly the original comics were awful to begin with so they could only go up from there no matter what they did in my opinion.
