You see, this original character, called MTS, is totally based upon Five Nights at Freddy's, but he is given an extremely varied group of forms based upon Riders from each series. They are based upon the Heisei Riders, but some of the Kamen Rider Series are harder to make forms for than others. In Series order, how hard are forms to make in each series? Let me see.
Kuuga: Rather hard, as there are no Secondary Riders.
Agito: Slightly easier.
Ryuki: Fairly easy, considering there are so many other Riders
Faiz: In the Middle.
Blade: So so.
Hibiki: Ehh.
Kabuto: Uhh, slightly hard
Den-O: Fairly easy
Kiva: Harder:
Decade: Easy
Double: Easy
OOO: Fairly Easy
Fourze: A bit harder
Wizard: Slightly hard
Gaim: Fairly easy.
Drive: Slightly easy
Ghost: Easy

Well, all of the Neo-Heisei Riders and Reiwa Riders are at least SOMEWHAT easy.

So, here are some of my ideas for some of his forms.

Decade: When it comes to my OC, he is very dependant on his powers and the powers of others. So calling his Driver the Dependriver not only fits with the naming convention. Decadriver and Diendriver, but also reflects his dependance on using his powers. As if he doesn't have his powers, he is pretty much an average human with average in everything.
W: He could give Double extra forms and an extra thought stream.
OOO: I think that Freddy Fazbear could be the Head Medal, the torso could be based upon someone else, and Foxy the Pirate Fox could be the Leg Medal.
Fourze: It could be easy to make new Astroswitches based upon FNaF
Gaim: Imagine the Five Nights at Freddy's Animatronic Heads as the Arms. Freddy Arms, Bonnie Arms, Chica Arms, etc.
Ghost: Come on. Imagine if MTS became a Parka Ghost and went onto Ghost.
Ex-Aid: I have thought of a whole fan-fiction for this. The form could be based upon the Security Guard.
Zi-O: I can see him using a new Form-based Ridewatch, but all of his powers and forms get stolen by an Another Rider
Zero-One: Considering that Zero-One is based upon Animals, Zero-One seems highly possible considering the animatronics.
