The Taka Medal Zero-One snagged at the start of the game flies off after he gets blasted by Zeus, where it then falls into the hands of Eiji.

As for why Lost Ankh was revived, I think it's safe to assume Zeus brought him back considering he brought every other Greeed back and he clearly forged new Red Medals anyway.
