I talked about this more in the second episode's write-up, but the tension between them is that Hana views Sakura as her "real family", while Sakura's use of that term for Yuriko accidentally demeans the bond that she shares with Hana. It's an accident, but it expresses the doubts Hana has that Sakura's as dedicated to their relationship as Hana is.
