I'll use the following picture as reference as its the last one i took and haven't changed anything:

My setup, one flash to the side, one flash above and a piece of white poster board to bounce the left. You could probably just do it with 1 flash above or to the side as well though.




Out of camera, it looks like this :



Then crop, remove dust, and a couple minor tweaks in lightroom. If you need the EXIF data, its all in the flickr link. I think it was around f/13 and 1/200s or so.
