This is in direct contrast with video games. In the video game world, death means absolutely nothing. You can always push the reset button and try again, no matter how many times you die. You may get a penalty of some kind but death ultimately means nothing.
In other words, you're correct. The Doctor Motif and the Video Game Motif don't go together. That's the point. The show forces you to look at Death from two completely different angles in order to examine it.
I'd say that's about the only thing the show does well with its Doctor/Video Game motif and really about the only thing it does to relate the two. Whenever it tries to emphasize these elements individually, you get stuff like its somewhat confusing game terminology or the character of the week subplots which is in my contender for some of the lamest subplots in Kamen Rider. Both motifs have enough ideas on their own to fill out an entire show but by trying to focus on them both at once, I can't help but feel both motifs bog each other down especially since, aside from the contrast between their ideas of death, the show doesn't do a good job of relating the two motifs.

Here's the thing. The fruit motif in Gaim was really just a gimmick in terms of the appearance. It's not like the show suddenly started talking about the existenstial nature of fruits.
